*If we do not take your insurance, you qualify for the Self Pay Plan, which is $175 per visit. Normally, a patient-physician relationship takes at least 2 visits, however there are cases that may take longer. This is determined by the physician on a case-by-case basis.

The total cost to get an Illinois Medical Card here in Illinois depends on your insurance, but not including doctor’s fees: $225 ($100 for Illinois certification for 1 year and $125 for background check, application processing, and photo) or total of $175 if you are on disability or are a veteran. To our knowledge, we are the only office that takes insurance for your physician visits.

By the way, this is a one-stop-shop for you to get your application sent to the Illinois Department of Public Health. It is also the most cost efficient. WE TAKE MOST COMMERCIAL INSURANCES.

  1. We do not make these diagnoses. They are done by your specialist or doctor, and our doctor reviews these records to make sure you have the condition. You must come to the appointment with your medical records in hand or faxed/emailed to us prior to your appointment. We will not certify you without your medical record stating you have one of the conditions.
  2. If you do not have any of the conditions above, please do not try to get the certificate. The state will reject you, and you will waste your time and money.
  3. Please be honest with our intake specialists. They are screening you to make sure you will not waste your time and money.
  4. Call us for a brief consultation if you have one of the above medical conditions. We make this process easy and painless!

After your physician certifies you, the state has been taking 40-45 days for certificates to come to you in the mail. You will be asked to see your doctor on a continuous basis, and we recommend seeing your doctor 4 weeks after you start using treatment.
